This section contains many of the important forms your council, assembly or circle is expected to complete during the fraternal year.

Forms are listed in order of deadline dates, but can also be sorted by report name or report number. Officers should review this site often to verify that the unit is filing reports on time.

Each unit is responsible for sending reports to the appropriate state/district officials if necessary. In every instance, copies of each report form should be kept for the council, assembly or circle files. Some forms allow officers to email the report to the Supreme Council and up to three additional people.
