Due to technical difficulties and after considerable deliberation, the Supreme Council has decided to remove access to the online submittable forms that were engineered using PDF formatting until a solution to the issue we are experiencing is reached. We sincerely apologize for this major inconvenience and ask that you continue to use the print forms. These forms can emailed to firstname.lastname@example.org. We want to assure you that a solution to the issue is actively being researched to restore full functionality at the earliest possible opportunity.

Tips when using online forms:
- Please print out a copy of the form before submission.
- To email a copy to yourself, include your email address at the bottom of the form.
Every council is responsible for making the report and sending them to the appropriate state/district officials if necessary. In every instance, copies of each report form should be kept for the council files.
