What is Form 365?
Form 365 is the Service Program Personnel Report. It notifies the Supreme Council of your council’s service program personnel.
How to submit Form 365
Before the new fraternal year, each council’s grand knight or financial secretary is responsible for completing and submitting the form to the Supreme Council shortly after the appointment of service program personnel and no later than June 30.
The Member Management application on Officers Online is the best and most efficient way to do this. By submitting digitally, you do not need to send a hard or emailed copy of the Report of Council Service Program Personnel form to Supreme Council.
You will also need to submit a copy of Form 365 to your district deputy. You can generate a PDF of the report and send by mail or email.

How to access Member Management
- If you do not have an Invitation Code, use the Account Assistant link under the SIGN IN button on the Officers Online Sign In page.
- Clicking Continue on that page will generate an email to you with the status of your account, and if not registered, instructions on how to register your account. If you do not receive an email within 24 hours, contact our Customer Service Department at (800) 380-9995.
- If you have an account registered and forgot your password, use the Password Reset link under the SIGN IN button on the Officers Online Sign In page. Clicking Continue on that page will generate an email to you with a temporary password, which you can use to reset your password. If you do not receive an email within 24-hours, contact our Customer Service Department at (800) 380-9995.
