Forms

The Application for Council Charter #137 must be completed within the first three months of forming a new council. The Fraternal Program Report Form (#10784) should be submitted as soon as possible after completing a program, on an ongoing basis throughout the fraternal year. The Notice of Round Table Coordinator (#2629) is only required for those councils who form a Roundtable.
